• This is NOT a referral form. You will not be required to submit large amounts of personal information

• We may contact you if we require further information to make a decision

• OR you may request a free 15 minute phone consult if you are unsure if or how we can support you

Step 2: Complete a REFERRAL FORM

• You will receive a referral form link if we are able to meet your needs listed on your intake form

• Once the referral form has been completed you will be contacted to schedule an initial appointment

• The initial appointment will not be scheduled unless the referral form has been completed
