Everything you need all in one place. If you haven’t already, register yourself using our online portal

  1. To log in you will need your email address (if you have multiple, it is the one Advocate OT contacts you on) & the password you used when registering

  2. If you don’t remember registering? That’s okay! Your email is in our system, simply click ‘forgot password’ and set yourself a new one

  3. If you are on the mobile website, you will need to navigate to the top right corner of your screen and click on the ⋮ button

On the client portal you can view and download a copy of

  • Your referral form

  • Your Home visit form

  • Your service agreement(s)

  • Your upcoming appointments

  • (more to come soon!)

*You cannot book or change appointments but you can cancel them.
